The Coconut Creek Police Department’s Persons with Disabilities Registry is available to individuals who currently reside in the City of Coconut Creek and who are diagnosed with a disability due to any of the following conditions: autism spectrum disorder, blindness, deafness, non-verbal, intellectual disability, people prone to seizures, dementia, or any other cognitive impairment. Registrants must provide a doctor’s diagnoses of the condition and proof of guardianship or Power of Attorney if registering someone else.

All participants of the program will receive an identification card and decal that will signify to a responding officer that the individual or residence they are encountering may involve a person with a disability. The Persons with Disabilities Registry decal should be placed on the front door or nearest window to the entrance of the residence and should be easily visible.

Frequently asked questions regarding registration in the Persons With Disabilities Registry database:

  1. Who will see the information on the database?Only Coconut Creek police personnel and emergency dispatchers will see the information in the database.

  2. What if I have more than one loved one I would like to register?Please do a separate registration for for each loved one.

  3. I'm having trouble entering information into the database. What should I do?Call our Public Information Officer, Scotty Leamon, at 954-480-7196 or send an email to the Community Services Unit

  4. What if some of the information I entered changes, such as our address or phone number?You can update the information in the database at any time by sending an email to the Community Services Unit

  5. What if I want my loved one taken out of the database?Call our Public Information Officer, Scotty Leamon, at 954-480-7196 or send an email to the Community Services Unit

  6. My loved one does not live in Coconut Creek but has a family member who lives there whom we frequently visit. Can I register him/her in the database since we come to Coconut Creek often?Certainly. Please enter the Coconut Creek address that is frequently visited as the loved one's home address so that this address is the one flagged for us when responding. Then add the fact that the loved one does not permanently reside at that address but frequently visits or stays the night there in the "additional notes or information" box in the "specific information" section of the registration page. As an example, the grandparents of an Autistic child live in Coconut Creek while their grandson lives in Coral Springs. The grandson stays with the grandparents often during the week when the child's parent(s) is working.
